Essential Requirements:
In-depth knowledge of person centred, asset based support planning and risk assessments including knowledge of safeguarding children and adults.
Awareness of factors that put people at risk of homelessness
Awareness of factors that put people at risk of hospital admission and / or delayed discharge
Skilled at IT and updating case management systems
Experience of working with a diverse range of people and working effectively in partnership with others
Level 3 NVQ qualification or equivalent work level experience.
The appointed Referral and Allocations Partner will:
1. Be person centred at all times and work with people to identify what is important to them and ensure the most appropriate intervention provides an effective route to support.
2. Deliver a ‘no wrong door’ approach, providing a supported referral into other services where more appropriate
3. Work directly with the Social Reablement team, allocating cases effectively to the most appropriate intervention
4. Work collaboratively with statutory and non-statutory services including but not limited to Hospitals, CMHTs, Substance Misuse Teams, Healthcare professionals, social prescribing initiatives, early help and preventative approaches of partner agencies, providing a coordinated and holistic service involving advocacy and crisis response.
The Role:
As a Referral and Allocations Partner will receive direct referrals and enquiries, providing a single point of contact for the Social Reablement and Crisis Support Service, working alongside partner agencies to ensure people accessing the service receive the correct support in a timely manner.
You will work in conjunction with the Home from Hospital Service ensuring the service is as easy to access as possible.
