Essential Requirements:
The successful applicant will:
- Have experience of working in mental health & wellbeing services with individuals who have forensic/offending history and/or complex needs
- Be educated to NVQ Level 3/4 or willing to work towards in Health & Social Care.
- Be flexible and able to work shifts including evenings, weekends and bank holidays (and waking night shifts if required)
- Have excellent verbal, written and IT skills.
The Role:
The service enables people to develop/maintain their independence, health and well-being through co-produced, asset based and recovery focused interventions and enabling individuals, whilst giving people the greatest possible control over their lives. The service will support clients with forensic/offending history and or have complex needs ensuring that people are at the centre and takes a holistic view of individual’s requirements.
The essence of this role is to provide support for people with complex needs and/or have a forensic history leaving acute mental health services. This will be delivered through a range of interventions such as telephone/face to face support and developing peer support.
About You:
- I will be person centred at all times and work with people to identify what is important to them and that recovery focused, asset based support is delivered to assist in people maintaining their wellbeing and reducing offending behaviours.
- I will empower and work in partnership support individuals to adhere to the conditions set out by the Ministry of Justice/Community Treatment Orders to prevent unnecessary clients stats or recalls into hospital.
- I promote independence, choices, dignity, privacy, respect and the participation of people, and their circles of support, in all that we do to ensure people have optimum control over their lives and support provided, reducing the reliance of paid support.
- I understand and work to CQC standards when delivering regulated activities at all times.
- I work with others to manage risk within the service, follow Safeguarding processes and Health & Safety requirements and report any concerns immediately to my manager
- I co-produce recovery focused, asset based support plans and positive risk assessments with clients that focus on abilities and aspirations.
- I offer outcome focused support, using recovery STAR, WRAP and any other tool useful to the client; I promote the use of technology where it supports a person.
- I provide safe, skilled support and advice around income maximisation, Personal Independence Payments, direct payments, brokerage, housing needs, budgeting, home maintenance, physical and emotional wellbeing.
- I provide a mix of social, emotional and practical support, taking my cue from the person, focusing on work/volunteering, education and training where the client needs and wishes exist.
- I will develop close partnerships and work in collaboration with other providers in the voluntary and statutory sectors that are important to the person and with their consent e.g. CHMT’s, Substance Misuse Team, Housing Teams and Wellbeing Centre’s.
